What is the United States Postal Service (USPS) 1583 form and why do I need to have it notarized?
- The U.S. Postal Service requires all commercial mail receiving agencies (CMRA), like PostScan Mail to collect a notarized USPS 1583 form from every user that opens an account. This form authorizes your mail to be opened, scanned, or shipped when it’s received at one of our locations. Without this form, the CMRA is required to return your mail to the sender.
- It ensures that only authorized parties, can access mail that is addressed to you.
- Form 1583 authorizes us to accept your mail on your behalf. It does not change your address with the U.S. Post Service or tell us where to forward your mail.
- Due to USPS regulations, you need to have the form notarized before sending it to us. The notary’s seal proves that you are the person who is authorizing PostScan Mail to receive mail in your name or your company’s name.
The information requested on this form is strictly used to complete USPS form 1583.
