How do I fill out the 1583 postal consent form?

To submit the 1583 postal consent form for yourself only, make sure to fill out the required areas (Sections 1 - 8). You can use our Postal Consent Form Wizard located in the settings of your account to automatically generate a form for you. We have included detailed instructions for each section if you would like to fill out the form manually.

 3. Your new PostScan Mail mailing address 

4.a. Mail Store name and address

5. Your name 

6. Your name 

7.Your residential address 

8. Two forms of I.D. 

If you are receiving mail for your spouse, make sure that you include their name in all of the required areas (Sections 2, 6, 8a, 8b and 16 of the 1583 Form) along with 2 forms of ID. Your spouse will also have to be present for the notary signing and their name included on the notary acknowledgment.

 If you have children under the age of 18 that you will be receiving mail for, make sure to include their names in the required areas (Section 5) with their name followed by "(minor)". Minors DO NOT require forms of ID. 

If you will be receiving mail for a business, make sure to include all of the business information in the required areas (Sections 2, 9, 10a, 10b, 10c, 10d, 10e, 11, 12, 13 and 14 of the 1583 Form) and proof of ownership. If you plan to receive mail for more than one business, you will need a separate 1583 form and proof of ownership for each business. Note: These additional 1583 forms do not need to be notarized as long as you have provided at least one notarized 1583 Form. If you are receiving mail for a web-based business, you will need to include "(Online)" next to the business name in the required area (Section 9 of the 1583 form). You will not need to provide proof of business, but we will require the web address of your online business. 

If you will be receiving mail for recipients in your account that are not your spouse, child or employee of your business, then we require a separate physical notarized 1583 Form for that recipient. (We require a physical notarized 1583 Form for each additional recipient that is listed in your account.)
