When completing USPS Form 1583 for a business entity, it’s important to include all required business details to comply with postal regulations and ensure mail handling authorization.


πŸ“„ Box 7 – Business Information

Box 7 of the USPS Form 1583 must include:

  • Business name
  • Business type (e.g., LLC, Corporation, Sole Proprietorship)
  • Physical business street address
    • If the business operates remotely and has no physical office, use the applicant’s home address
    • Do NOT use the mailbox address here
  • Business phone number
  • County and state of registration
    • Or the location of the business’s Registered Agent


πŸ‘₯ Additional Mail Recipients

If your business has:

  • Multiple officers, employees, managers, or partners who will receive mail
  • Multiple DBA (Doing Business As) names

Please list those names under Line 12 – Exceptions for Additional Recipients of Mail.

Employees do not need their own Form 1583, but:

  • They may be asked to supply two forms of identification if requested by the Postal Service.
